Financial Policy
The Clinic is a provider for many third party insurance carriers, Medicare and Medicaid. Please check your plan documents or call our office to see if we participate in your plan.
While we will assist you by filing claims, your insurance is a contract between you and your insurance carrier. You, not your insurance carrier, are responsible for your bill.
Insurance co-payment, if required by your plan, and proof of eligibility
is required at the time of your visit. Payment for services not covered
by insurance or other third party payer is due at the time of service. All
elective procedures not covered by insurance must also be paid in full before
the procedure is done.
The clinic utilizes Doctor Assistants to provide care for patients. Some
insurance companies will not pay for services provided by Doctor Assistants.
It is your responsibility to know the limitations of your plan benefits.
Economically Disadvantaged
The Clinic has established a sliding scale for those patients who are economically
disadvantaged. Patients are required to have a Financial Screening with
a Salina Family HealthCARE representative prior to being offered reduced
rates. Full payment of your account is expected from you after 30 days.
If you are unable to pay the entire balance within 30 days, your balance
will be transferred to an Accounts Receivable agency. It is your responsibility
to contact the Accounts Receivable agency to make payment arrangements.
Financial Screening
Salina Family HealthCARE Center provides quality healthcare to all people, regardless if their ability to pay. Assistance
can be provided if you have no insurance and are experiencing financial
difficulty paying for services.
Medicare
Our Doctors accept assignment for Medicare services and payment for our
charges will be made directly to Smoky Hill. You will be responsible for
the "Patient Responsibility" portion of the fee, as outlined
on your Medicare Explanation of Benefits.
Your are also responsible for any services deemed "non-covered"
by Medicare.
We will also file secondary insurance for services, provided
we have a copy of the explanation of benefits from your primary carrier.
A detailed description of the Financial Account Policy is available for
all patients upon request. Please contact the Billing Department with any
questions concerning your account.
