New Patient Forms are listed on the right. These forms may be printed, filled out and brought to your appointment.

 

Dental Clinic

and Fridays 8:00am to 12:00pm.

If you are unable to keep your scheduled appointment, please kindly give 24 hours notice to reschedule. Please arrive 10 minutes prior to appointments at both the medical and dental clinic. We ask that you arrive 30 minutes prior to procedures at the medical clinic.

 

For your convenience, patient parking is located on the east side of the building and west across the street behind Elmore Center.
